PALLIATIVE CARE

Palliative care is a specialized approach to medical care that focuses on providing relief from the symptoms, pain, and stress of a serious illness. It aims to improve the quality of life for both patients and their families by addressing physical, emotional, social, and spiritual needs.
While palliative care focuses on improving quality of life throughout the course of a serious illness, it also includes end-of-life care for patients who are nearing the end of life. Palliative care teams provide support to patients and their families during this time, addressing physical comfort, emotional support, and spiritual care.

CONTINUOUS OR HYBRID CARE

In a continuous care model, healthcare services are provided by a single provider or a closely coordinated team throughout the patient’s illness or healthcare journey. This model emphasizes continuity of care, with the goal of building long-term relationships between patients and their healthcare providers.
The primary care provider or care team coordinates all aspects of the patient’s care, including referrals to specialists, follow-up appointments, hospital admissions and discharges, and communication with other healthcare providers involved in the patient’s care.

MEDICAL & HEALTH CARE

Medical and healthcare encompass a broad range of services, practices, and disciplines aimed at promoting, maintaining, or restoring health and well-being.
Primary care serves as the first point of contact for individuals seeking healthcare services. It includes preventive care, health education, routine screenings, management of acute and chronic conditions, and referrals to specialists when necessary. Primary care providers may include general practitioners, family physicians, internists, and pediatricians.

SENSORY IMPAIRMENT CARE

Caring for individuals with sensory impairments, such as vision or hearing loss, requires a compassionate and patient-centered approach.
Begin by assessing the individual’s specific sensory impairment, including the degree of impairment, any underlying conditions, and the impact on their daily functioning. Understanding their needs and challenges is crucial for providing tailored care.
For individuals with vision loss, use verbal descriptions and tactile cues to convey information. Speak clearly and directly to the person, and provide assistance with navigating their environment, reading documents in accessible formats (such as braille or large print), and using assistive devices like screen readers or magnifiers.
